Comments
This inspection was conducted in response to a request for a new Food Service Operation License and to complete a facility review.
The operator has completed all items required from the previous inspection and has been approved to be licensed.
A signed application and payment for the amount of $428.00 for license #JSMH-9VEKSL was received at the time of inspection.
A Final Building and Fire Inspection was provided at the time of inspection.
Proof of Person In Charge training was provided at the time of inspection.

Note:
The shelving over the food preparation cooler is acceptable for now but, will be required to be replaced with commercial grade shelving is not maintained clean or in good condition.

Provide a Certificate of Occupancy once it has been provided by the Building/fire department.

Okay to operate.

License will be available for pick-up at the Kent Health Department once processing has been completed.
