3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed residue on the interior surface of the ice machine at the time of inspection.
Nonfood-contact surfaces of equipment shall be kept clean. Clean the residue from the interior of the ice machine and provide regular cleaning that maintains all equipment clean to sight and touch.
3717-1-06.1(M) / Outer openings - protected.
Corrected During Inspection Observed 2 open doors at the time of inspection without proper protection against the entry of insects and rodents.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. To protect against the entry of pests, keep all doors to the facility closed, or provide an approved method for excluding pests for these doors. PIC closed doors at the time of inspection.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing signs posted at handwashing sinks used by employees in both bathrooms, and the handwashing sink next to the front service area at the time of inspection.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

Observed employees properly washed hands at appropriate times and used a metal stem thermometer to measure internal food temperature during preparation. Person in charge requested to be licensed as a risk class IV operation in order to reheat in bulk. A critical control point inspection shall be conducted for this risk class IV operation.
