3717-1-02.4(C)(14) / PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Critical Repeat Person in charge did not ensure their employees are properly trained in food safety. Observed multiple critical violations at time of inspection (date marking, cleaning, raw storage, hand washing, sick policy, etc.). Education was provided to improve food safety.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Employees are not informed in a verifiable manner of their responsibility to report information about their health. The written sick policy was not signed by all employees. The PIC stated that all employees would sign the policy as soon as possible.

3717-1-02.2 (C ) / Hands and arms: when to wash
Critical Food employees did not wash hands when required. Employee contacted RTE foods immediately after adjusting mask with same gloves during food preparation. Education was provided to improve hand washing and glove usage.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Food not properly protected from contamination by separation, packaging, and segregation. Raw eggs were stored over RTE foods in the walk in cold holding unit. Education was provided to improve storage procedures.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Multiple pieces of equipment are not clean (deli slicer, can opener blade, dough equipment, pizza pans, mixer). The PIC stated that equipment would be cleaned immediately.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked (ham). Education was provided to improve date marking procedures.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
An employee drink was stored over the preparation area without a lid.

3717-1-06.2(E) / Handwashing signage.
Handwashing signs not posted at both hand washing sinks in the kitchen.

3717-1-06.1(M) / Outer openings - protected.
Outer opening not properly protected from pests due to gaps at the bottom of the rear exterior door.

3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Presence of dead insects (flies). There is a very large number of dead flies near the food preparation sink hanging from sticky tape.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Multiple pieces of equipment are not clean (microwave, tables, cold holding equipment, cooking equipment, storage racks and shelving

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. The floors of the entire facility are not clean (basement, walk in cold holding unit, bathroom, mop closet, and kitchen). The ceilings are not clean near the exhaust hood. The walls are not clean near the dough equipment and near the cooking equipment.

Repeat Unnecessary or nonfunctional items stored in the basement (old equipment, old doors, and excess paint).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There is a light bulb not working over the deli slicer. There are multiple areas of the kitchen in disrepair (flooring, walls, coved base, etc.).

3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Ventilation system not maintained clean (exhaust hoods over cooking equipment).

3717-1-04.2(D) / Ventilation hood systems - adequacy.
Repeat Ventilation hood system not sufficient to prevent grease buildup along the nearby walls, ceilings, and equipment.
