3717-1-05.1(L) / Handwashing sinks - location and placement.
Critical Repeat Observed there was no handwashing sink located to allow convenient use by employees behind the left side bar at the time of inspection.
To ensure frequent and proper handwashing, a handwashing sink shall be located to allow convenient use by employees in food preparation, food dispensing, and warewashing area, and in, or immediately adjacent to, toilet rooms.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection at the time of inspection between the sewage system and a drain originating from equipment (ice machine) in which food (ice) was placed.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Observed single-service and single-use articles (beverage containers at front storage) were not properly stored at the time of inspection.
Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least six inches (fifteen centimeters) above the floor
3717-1-06.4(A) / Repairing.
Repeat Observed tiled flooring behind the left side bar were not being maintained in good repair at the time of inspection.
The physical facilities shall be maintained in good repair. Repair flooring behind the left side bar so that it is smooth, durable, and easily cleanable.

Observed equipment food-contact surfaces of ice bins behind both bars were dirty at the time of inspection.

3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on floors throughout the facility at the time of inspection.
To reduce sources of contamination that may attract pests, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.2(B)/Handwashing cleanser - availability.
Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink for the right side bar at the time of inspection.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. Correct By: 19-Dec-2014
3717-1-05.1(E)/Backflow prevention device - design standard.
Observed no backflow prevention device installed on a water supply system where a hose was connected to the water supply outlet of the service sink at the time of inspection.
To prevent contamination, a backflow or backsiphonage prevention device installed on a water supply system shall be installed that meet American Society of Sanitary Engineering (ASSE) standards and as referenced in the Ohio plumbing code for installation, maintenance, inspection, and testing. Comments
Observed facility appeared significantly cleaner at the time of inspection.
Scoops were purchased for properly serving ice in drinks, ice bins were cleaned and sanitized, and a sanitizer test kit was purchased for measuring sanitizer concentration.
Handwashing sink was supplied with hand cleanser, and the service sink water supply outlet was equipped with an approved backflow prevention device.
Continue to make progress correcting all remaining violations. Progress will be checked at the time of the next standard inspection.
