[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(B)(2)(k) / PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
Critical PIC unable to demonstrate knowledge of cleaning and sanitizing of utensils. PIC stated the utensils stored on wax paper on the prep line are cleaned and sanitized daily. PIC was informed utensils that are used for TCS foods like meatballs, chicken, guac are required to be warewashed at minimum every 4 hours.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed shamrock farm milks held in the coca-cola cold holding unit above 41 degrees F (52). PIC voluntarily removed product from sale and will be discarded.

3717-1-05.1(J) / Backflow prevention device - when required.
Critical No approved backflow prevention device on the plumbing system for the ice tea dispensing equipment at the serving area.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Improper storage of food items. Observed perforated bag of pre-washed spinach stored directly on the shelf in the walk-in cold holding unit. PIC will ensure employees are keeping packages in the original box or place in containers.

3717-1-02.3(C) / Hair restraints - effectiveness.
Corrected During Inspection Food employee hair not effectively restrained. Observed employee without an effective hair restraint while working with clean items and food. Employee put on a hat and effectively restrained their hair.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed utensils on the prep line used for the meatballs, chicken, and guac being placed on soiled wax paper. PIC removed wax paper and will place directly on prep table which can be properly cleaned and sanitized at least every 4 hours.

3717-1-04.8(A) / Equipment and utensils - air-drying required.
Corrected During Inspection Equipment and utensils are not being air dried. Observed salad bowls on the prep line, container used to store utensils on the prep line, and containers under the back prep table stacked wet after cleaning and sanitizing. PIC removed containers and will inform employees to ensure proper air drying occurs before storing or using the equipment.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or single-use article not handled, displayed, or dispensed properly. Observed beverage containers displayed exposed on the front service counter.

3717-1-04.4(F)(2) / Warewashing sinks - cleaning and sanitizing.
Food preperation sink (also used to pre-soak dirty utensils and equipment) not properly cleaned and sanitized. Observed colinder equipment in the sink and the bottom and walls of the sink with an accumulation food debris.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors in the facility specifically around the grease trap not maintained clean.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Light intensity less than ten foot candles in required areas. Observed measured light intensity of 4 foot candles in the walk-in cold holding unit.

Comments
Ensure new employees are informed in a verifiable manner of their responsibility to report information about their health. Observed signed reporting agreement for employees hired before May 2018, if additional employees have been hired ensure they are being trained on the symptoms, illnesses, and exposure that need to be reported to the person-in-charge (PIC).
Observed water near the water heater and PIC stated the water is from the mops. Ensure water heater is not leaking and mops shall be properly air dried.

The violations documented shall be corrected and will be checked during the follow-up inspection.
