3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Quaternary ammonium sanitizing solution at incorrect concentration at the three-compartment sink. Observed equipment and utensils soaking in the compartment at a concentration of 25-50 ppm. PIC drained and refilled the compartment with the proper concentration.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods (sliced tomatoes) not being cold held at the proper temperature in the front prep refrigeration unit. PIC removed the tomatoes from the prep line.

3717-1-05.1(J) / Backflow prevention device - when required.
Critical Repeat No approved backflow prevention device on the plumbing system for the ice tea dispensing equipment at the serving area. Observed shut-off type valve on the water supply line. Please provide documentation on the device installed if it is rated for backflow prevention (ASSE 1022).

3717-1-03.2(G) / Washing fruits and vegetables.
Raw vegetables not washed or washed improperly. Observed employee using the slicer without washing cucumbers and other vegetables at the time of inspection.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Repeat Single-service or single-use article not handled, displayed, or dispensed properly. Observed beverage containers displayed exposed on the front service counter.

3717-1-04.4(F)(2) / Warewashing sinks - cleaning and sanitizing.
Food preperation sink (also used to pre-soak dirty utensils and equipment) not properly cleaned and sanitized. Observed colinder equipment in the sink and the bottom and walls of the sink with an accumulation food debris. The sink shall be cleaned and sanitized prior to washing produce.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Corrected During Inspection Plumbing system not properly maintained or repaired. Observed the front hand sink with low water pressure. PIC removed the aerator and removed the debris. Obsered the water pressure restored to the hand sink.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors in the facility specifically around the grease trap and baseboards not maintained clean. Damaged floor tile in the dish area shall be repaired if flooring is no longer easily cleanable.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Repeat Corrected During Inspection Light intensity less than ten foot candles in required areas. Observed measured light intensity of 3-5 foot candles in the walk-in refrigeration unit. PIC changed out the light bulb and the light intensity was measured over 20+ foot candles.

Ensure handles to equipment are being cleaned frequently and are maintained clean.
Equipment such as containers and bowls shall be air dried before stacking to prevent "wet stacking".
The violations documented shall be corrected and will be checked during the follow-up inspection.
