[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed a directly connected drain on the 3-compartment sink used for washing fruits and vegetables (bananas and tomatoes) at time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Provide a properly designed food prep sink.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed non-food contact surfaces of equipment with an accumulation of food debris and dirt at the following locations at the time of inspection: 1) walls of walk-in cold holding unit. 2) floors in the walk-in freezer.
To prevent health hazards, nonfood-contact surfaces of equipment shall be kept clean. Clean and maintain the equipment surfaces listed above. Bottom shelves in walk-ins shall be maintained up off the floor (atleast 6 inches).
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Repeat Observed the plumbing system on the water filters was not properly maintained near the back warewashing room at the time of inspection. A fitting for the plumbing system was leaking water onto the floor.
To prevent health hazards, a plumbing system shall be properly maintained. Repair the plumbing system listed above.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Observed improper draining of condensation and other nonsewage liquids for the walk-in cold holding unit and for the ice bin next to the salad prep cold holding unit (not in use) at the time of inspection. Condensation lines were missing and drain into buckets.
To prevent health hazards, condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Provide proper drainage for the walk-in unit and ice bin (air gaps and directed to floor drains).
3717-1-06.1(M) / Outer openings - protected.
Observed an outer back door without proper protection against the entry of insects and rodents (not self-closing) at the time of inspection.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. Install door to be self-closing.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of grease and black substance on ventilation hood above the Nieco conveyor equipment at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the hood covers and area above equipment. Per discussion with owner, the hood is professionally cleaned 3-4 times a year.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and food debris on the floors and wall behind the ice machine and the floors and wall around the washing machine at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean and maintain the floors listed above.

A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO (ice machine, coffee machine, slushy machine, etc.).
Ensure the catch trays for the conveyor broiler are warewashed every 4 hours.
Drainage lines off ice machines (bin and condenser) shall be separate and contain air gaps.

The violations documented shall be corrected as soon as possible and will be checked during next standard inspection.
