3717-1-03.2(G) / Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. Unwashed tomatoes are stored in the reach in cold hold holding unit and there is not a proper sink in the FSO to wash the tomatoes.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Multiple pieces of non-commercial equipment in the food service (microwave, toaster, etc.).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Floors throughout the building are not clean (basement, bathroom, bar, walk in cold holding unit, and in the kitchen under equipment).
