Repeat Facility does not have an employee with manager certification in food protection.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Nonfood-contact surfaces not easily cleanable. Exposed wood shelving under the steam table. Cabinets throughout showing deterioation and in poor condition. Observed laminate peeling for the prep surface.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed shelving under the hot holding table not maintained clean. Continue to work with maintenance to ensure the facility is pest free and continue to maintain the facility clean.
