[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Repeat No written procedures for responding to vomiting or diarrheal events.

3717-1-03.4(E) / Cooling methods - temperature and time control.
Improper method for cooling TCS foods. Observed container of cooked rice stored next to the grill at the time of inspection. PIC stated this was cooked earlier and is still cooling down and then will be placed in the refrigerator. Ensure food is being rapidly cooled by an approved method (shallow pans with lid cracked for venting). If rice is going to be held outside of mechanical hot or cold holding a 4-hour hold procedure (tilt procedure) must be developed and implemented for the product.

3717-1-03.4(C) / Thawing - temperature and time control.
TCS foods not properly thawed. Observed partially frozen beef thawing at room temperature near the dry storage rack. Thawing shall occur under refrigeration, submerged under cold running water, during the cooking process, or microwave if cooked immediatly afterwards.

3717-1-03.2(M) / Wiping cloths - use limitation.
Improper use and/or maintenance of wiping cloths. Observed soiled wiping clothes being stored on the countertops. Observed prep sink filled with dirty wiping clothes.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Non-food contact surfaces of equipment are unclean. Observed build-up of grease and debris on multiple pieces of equipment at the cook and prep line (rice warmers, prep tables, lids, etc.). Observed in-use utensil containers not maintained cleaned (clean and sanitize every 4 hours).

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed odor from sinks and grease trap area. Ensure grease trap is being maintained. Observed liquid spill staining near the grease trap and plumbing in that area.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors, walls, and area around the grills and the side wall/corner under the hood system not maintained clean. Observed grease building up on the white tank and the ceiling above the tank in between the hoods. Observed grease build-up on the hood system in the corner and PIC stated when it rains water drips onto the floor in that area.
