[?] A summary of the violations found during the inspection are listed below.

3717-1-03.1(L) / Package integrity - specifications for receiving
Critical Observed food (dented cans of: tomato sauce, clams, great northern beans) at the time of inspection in packaging that does not appear to protect it from contamination.
To prevent adulteration, food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed food items (butter, jellies/jams, dressing, pastries in storage under raw bacon and raw shell eggs) that were not properly protected from contamination by separation, packaging, and segregation at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items (box of bread stored on the floor of the walk-in freezer) at the time of inspection.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (multiple reach-in freezers, 2 chest freezers, microwave) at the time of inspection that was not approved by a recognized testing agency.
Food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Remove all equipment that has not been approved by a recognized food equipment testing agency.
3717-1-04.1(Y) / Temperature measuring devices.
Observed cold holding equipment (reach-in cooler) did not contain a thermometer, or the thermometer was not correctly placed at the time of inspection.
To ensure TCS food are held at an acceptable temperature within this equipment, a properly designed temperature measuring device as specified in this rule shall be located in the warmest part of the reach-in cooler.
3717-1-06.1(A) / Floors, walls, and ceilings.
Observed the floor of the walk-in cooler was not smooth and easily cleanable.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed the floor and wall junctures outside the walk in freezer and inside the walk in cooler were not properly coved at the time of inspection.
Exposed floor and wall junctures are hard to clean adequately and may trap food debris that could attract pests. The floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on floors throughout the facility (between cooking equipment and wall, interior surfaces of the walk-in cooler, back dry storage room) at the time of inspection.
To reduce sources of contamination that may attract pests, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed the vent covers and other surfaces for the ventilation system above cooking surfaces were not being properly cleaned at the time of inspection.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination.
