[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed no culinary sink with an air gapped drain line at the time of inspection for the washing of fruits and vegetables. Operation washes fruits and vegetables in the far left basin of the 3 compartment sink with a strainer.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide a culinary sink with air gapped drainage for washing fruits and vegetables.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items in the walk-in cold holding unit at the time of inspection. Boxes of chicken wings and corned beef were being stored on the floor.
To prevent contamination, food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Food items shall be stored 6 at least 6 inches off the floor.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed unapproved containers (gladware, re-using single use containers) used for storing food that was not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Discontinue using unapproved containers and re-using single use containers.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed non-food contact surfaces of equipment (popcorn machine, cookie display case, scale by fryers) dirty at the time of inspection.
To prevent contamination, nonfood-contact surfaces of equipment shall be kept clean. The equipment listed above shall be cleaned and maintained.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed cleaned equipment (plastes, bowls, etc.) not properly stored on the storage rack near the cook line at the time of inspection.
To prevent contamination, cleaned equipment and utensils shall be properly stored in a self-draining position that allows air drying and covered or inverted. Discontinue not properly storing plates, bowls, etc. on the storage rack.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Observed single-service or single-use articles (large bar straws) were not displayed or dispensed properly at the time of inspection.
To prevent contamination, single-service articles or single-use articles shall be displayed and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule. Large bar straws shall be stored and dispensed properly. Recommend using a dispenser or purchasing wrapped straws.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Observed improper draining of condensation from the walk-in cold holding unit at the time o inspection. A container is being used to collect the water off the condensor.
To meet the requirements of this rule, condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Install a drainage line off the condensor with a proper air gap that is directed to a floor drain.
3717-1-06.4(A) / Repairing.
Repeat Observed multiple floor tiles under the cooking line and under the ice machine to be damaged or missing at the time of inspection.
To allow for easy cleaning, floors of a FSO shall be smooth, durable and easily cleanable. Repair the flooring to meet the requirements of this rule. The physical facilities shall be maintained in good repair.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris at the following locations at the time of inspection: 1) floors under the cooking line. 2) floors under the ice machine 3) other hard to reach areas within the facility.
To prevent contamination and possible pest infestation, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean the above locations and maintain them clean by creating a cleaning schedule/log.
