This FSO facility has undergone a final pre-licensing inspection and has been approved to be issued a risk level IV license.
A signed application and license fee was submitted to the Kent City Health Department. Final Building and Fire approval along with a Certificate of Occupancy was provided.

Okay to Operate.

The following shall be completed by the 30 day inspection to avoid possible violations:

1) Condensation drainage lines from the walk-in freezer and walk-in cooler shall be draining to a sanitary sewer and not directly to the parking lot.
314.2.1 Condensate disposal. Condensate from all cooling coils and evaporators shall be conveyed from the drain pan outlet to an approved place of disposal. Such piping shall maintain a horizontal slope in the direction of discharge of not less than one-eighth unit vertical in 12 units horizontal (1-percent slope). Condensate shall not discharge into a street, alley or other areas so as to cause a nuisance.
2) Provide a diarrheal and vomiting clean up procedure and proper kit.
3) Provide an verification that managers have trained all food employees on the illness policy and illness reporting requirements.
4) Provide a proper consumer advisory on the in house and to go menu. Current advisory is missing the disclosure. ensure that each food item that be be served with raw or rare foods to have a *.

Disclosure shall include:
· A description of the animal-derived foods, such as “oysters on the half shell (raw oysters),” “raw-egg Caesar salad” and “hamburgers (can be cooked to order),” or…
· Identification of the animal-derived foods by asterisking them to a footnote that states that the items are served raw or undercooked, or contain (may contain) raw or undercooked ingredients
