Critical Corrected During Inspection Observed build-up on the ice shoot in the dinning room. Person-in-charge cleaned and sanitized the ice shoot.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed a torn gasket on the walk-in cooler door. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed grease build-up on the sides of the fryers, Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Repeat Observed the presence of dead flies in light lenses in the back room.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
