[?] A summary of the violations found during the inspection are listed below.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. As of 8/28/2018 GM was awaiting her test results from her Level II Training
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Corrected During Inspection Observed multiple coolers without a thermometer. A food thermometer was not readily accessible.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed torn gaskets on the big walk in cooler and on the small prep cooler under the potato wedge station. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed accumulation of debris on multiple cooler door gaskets. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed buildup of debris on the vents in both the mens and womens restroom. The ventilation system was not being properly cleaned or was creating a public health hazard.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
