Critical Repeat Equipment food-contact surfaces or utensils are unclean. Observed buildup of debris on food contact surfaces. Containers were placed in the 3-bay sink to be rewashed.

Critical Corrected During Inspection Observed TCS foods not date marked. PIC date marked the food during the inspection. Refrigerated, ready-to-eat, TCS foods not properly date marked.

Critical Observed live ants on the slushy machine and throughout the facility. On 3/15/24, PIC had a pest control company come out and treat for ants. PIC also cleaned out the slushy machine on 3/15/24. Monitoring is to be continued. Presence of live insects, rodents, and other pests.

Observed the warming rack smoking with a burning smell coming from the unit. PIC turned off the unit during the inspection. Physical facilities not maintained in good repair.

Comments
Facility voluntarily closed over the weekend to clean. Van and Schelby walked through the facility as part of the on-going action plan on 3/18/24. Cleanliness of floors and coolers did show some improvement, but violations were still observed. Timers were observed being used for handwashing and utensil cleaning. Due to the improvement of cleanliness, the facility was given clearance to reopen as long as the facility cleans all food contact surfaces and non-food contact surfaces prior to opening. The ant infestation requires continuing monitoring to ensure ants are not getting into any food items or equipment; a pest control company was at the facility on 3/15/24 for pest control services. Van was also at the facility on 3/15/24 to ensure the slushy machine was properly disassembled for cleaning. The walk-in freezer had gone down again during the night of 3/14/24. A service contractor repaired the freezer on 3/15/24 and the facility was asked not to use the freezer until it had been inspected by Marion Public Health. The freezer was checked during the follow up inspection on 3/18/24 and had an ambient air temperature of -20°F. The facility was given clearance to use the freezer.
