Critical Observed live ants in the liquid mix inside of the slushy machine as well as throughout the facility. PIC is to not use the slushy machine until it has been emptied and cleaned. PIC is also to contact a pest control company to come out to control the ant infestation. Presence of live insects, rodents, and other pests.

| The walk-in freezer's compressor had been repaired the morning of the inspection by a service contractor; ambient air temperatures in the freezer were reading around 15°F. No food was in the walk-in freezer at the time of inspection. PIC was cleared to use the walk-in freezer. Van and Schelby walked through the facility as part of the on-going action plan. Cleanliness of floors and coolers did show some improvement, but violations were still observed. Timers were observed being used for handwashing and utensil cleaning. Cleaning logs were still not being consistently completed by each shift. |
