Corrected During Inspection Observed no towels or drying device at the handwashing sink by the single use items storage. PIC put some paper towel.

Repeat Observed excess ice build up inside and by the door of the walk in freezer. Observed condensation on the ceiling of the walk in freezer. Equipment components are not intact, tight or properly adjusted.

Repeat Observed build up and debris in the gaskets and inside coolers and freezers throughout the facility. Observed buid up on the shelves of the walk in cooler and dust build up on the vent covers of the walk in cooler. Non-food contact surfaces of equipment are unclean.

Observed standing water on the floor by the 3 compartment sink. Floor drain needs to be fixed. Plumbing system not properly maintained or repaired.
