Critical Repeat Corrected During Inspection Observed build up in the ice machine. PIC cleaned it during inspection. Equipment food-contact surfaces or utensils are unclean.

3717-1-03.4(D) / Cooling - temperature and time control.
Critical Repeat Corrected During Inspection When asked PIC what they do with their leftover of meatball at the end of the day, PIC stated they cool it down and put it in the walk in cooler. Facility is a Risk Level III and is not allowed to cool and reheat food items. Tlaked to PIC about discarding any leftover of meatball at the end of the day. Improper cooling of TCS food.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed damaged gasket on the lid of the prep table at the front. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build up and debris on the shelves of the walk in cooler and in the 2 door reach in cooler at the front. Non-food contact surfaces of equipment are unclean.
