Critical The current health policy was not able to be verfiable. Person-in-charge was provided a form they can have every employee read and sing. Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Observed build-up on the ice shoot on the drive thru drink machine. Person-in-charge cleaned the ice shoot. Equipment food-contact surfaces or utensils are dirty.

Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.

Repeat Observed a damaged gasket on the walk-in freezer and a torn gasket on the walk-in cooler. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Observed a damaged door handel on the pepsi cooler. Equipment and/or components are not maintained in good working order.

Observed no handwashing sign posted at handwashing sink used by employees in the kitchen.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

Observed an employee coat stored on a shelf with food.

3717-1-06.4(A) / Repairing.
Repeat Observed damaged/ or missing floor tiles and baseboards in the kitchen. Also water damaged ceiling tiles in the dinning room. The physical facilities are not being maintained in good repair.
The physical facilities shall be maintained in good repair.
3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Repeat Observed dead flies in the light lenses in the kitchen.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
