Repeat Did not observe a current ODH Manager Certificate at the time of inspection. Facility does not have an employee with manager certification in food protection.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Repeat Observed degraded seal along the 3-bay sink. Fixed equipment not properly sealed or spaced for cleaning.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Observed build-up of debris in prep coolers and freezers. Non-food contact surface(s) not cleaned at the required frequency.

Repeat Observed build-up of debris on floors and walls around 3-bay sink as well as on ceiling vents. Facility not maintained clean.

3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Observed build-up of debris in ice maker. Equipment food-contact surfaces or utensils are unclean.
Comments
Received a complaint on 8/24/23 from a former employee about concerns regarding cleanliness, employees not wearing gloves, and the facility not doing dishes until the end of the night. During the follow up inspection, employees were observed wearing gloves. A few dirty dishes were observed in the 3-bay sink waiting to be washed, but not an unreasonable amount. The floor in the kitchen was observed to still be damp from cleaning. A build-up of debris was still observed on the walls and floor around the dishwashing area, which was violated during the 8/22/23 standard inspection. The PIC was reminded to have this area cleaned. No critical violations were observed during the follow up and complaint investigation.
