Critical Corrected During Inspection Observed no employee health policy at the time of inspection. PIC was given a policy to implement. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Observed missing thermometer in the prep cooler by the two bay sink and missing thermometer in the cooler above that prep table at the time of inspection. PIC placed a thermometer in both. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-03.2(R) / Food storage - prohibited areas.
Corrected During Inspection Observed salad and pie uncovered and stored in the bottom of a prep cooler at the time of inspection. PIC removed and disposed of both items. Food stored in prohibited area.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Corrected During Inspection Observed ham slices in the walk in cooler without protection from contamination. PIC placed lid on top of it at the time of inspection. Improper storage of food items.

3717-1-03.2(Y) / Miscellaneous sources of contamination.
Corrected During Inspection Observed personal employee items being stored in the upright McCall cooler by the grill line at the time of inspection. PIC removed both items. Food not protected from contamination.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed ceiling vent covers throughout the facility with an accumulation of dust at the time of inspeciton. Observed vent cover inteh upright McCall cooler by the grill line with an accumulation of dust. Non-food contact surfaces of equipment are unclean.

Observed a build up of debris in the Alto-shama warmers at the time of inspection. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed two nonfunctional microwaves at the time of inspection. PIC stated he has a order in to replace them both. Nonfunctional items and /or litter on premises.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed missing coving on the corner by the walk in cooler at the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.
