Repeat Observed excess ice build up inside and by the door of the walk in freezer. Observed condensation on the ceiling of the walk in freezer. Equipment components are not intact, tight or properly adjusted.

Repeat Observed torn gaskets on the door of the ice cream toppings cooler. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build up and debris in the gaskets and inside coolers and freezers throughout the facility. Observed buid up on the shelves of the walk in cooler and dust build up on the vent covers of the walk in cooler. Non-food contact surfaces of equipment are unclean.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Repeat Observed standing water on the floor by the 3 compartment sink. Floor drain needs to be fixed. Plumbing system not properly maintained or repaired.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed build up and debris on the floor and underneath equipment throughout the facility. Facility not maintained clean.

Comments
Facility new dishwasher is still not working. PIC stated Ecolab was at the facility after the last inspection to check the dishwasher but it is still not working. Facility cannot still use the dishwasher until it is fixed and working properly. PIC will have Ecolab come back and check it again. Facility shall contact the Health Department once the dishwasher has been fixed for another inspection. Facility shall use their 3 compartment sink in the meantime.
