3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed several crack and broken plastic dishes. Once they crack or break they need replaced because proper sanitation can not occur. The dishes will continue to crack and can lead to additional problems.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Comments
Complaint stated she found a piece of clear plastic in her burger. She purchased food around 8:30pm on 3/31/16. She called the store and corporate number and they only wanted to replace her food.

Spoke to the manager, He was aware of the complaint and stated he has had two similar complaints of this issue.

Conducted an inspection. I did observed a few cracked or broken plastic dishes. This was also violated on their last inspection on 12/22/15. These broken dishes need thrown away and replaced.
