Critical Repeat Corrected During Inspection Observed the ice machine with some build up. PIC cleaned on site. Equipment food-contact surfaces or utensils are unclean.

3717-1-03.4(D) / Cooling - temperature and time control.
Critical Corrected During Inspection Facility is currently cooling and reheating their meatballs. The facility is currently a level III facility and their license does not allow them to cool and reheat. After speaking with the PIC, she stated that the meatballs will be thrown away at the end of the night.

3717-1-04.8(A) / Equipment and utensils - air-drying required.
Repeat Observed the dished being wet stacked and not able to properly drained. Equipment and utensils are not being air dried.
