3717-1-02.1(A) / Employee health reporting
Critical Employee Illness Policy not available. Please provide employee illness policy that lists the 13 food borne illnesses referenced in OAC 3717-1-2.1.
To prevent the transmission of foodborne illnesses, the license holder shall require food employees and conditional employees to report to the person in charge information about their health as it relates to diseases that are transmissible through food.
3717-1-06.2(B) / Handwashing cleanser - availability.
Critical Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink by the 3 comp sink. PIC changed batteries of soap dispensor, but it will not work. It might need to be replaced.
To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.

3717-1-02.2(G) / Jewelry - prohibition.
Corrected During Inspection Observed food employees wearing ring with stone placements during food prep.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands.
