Critical Observed build up of mold in ice machine. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical When asked PIC how often the nozzles of the drink machine are cleaned she stated once a week. Equipment contacting non TCS food shall be cleaned at least every 24h. Food equipment surfaces are not cleaned at the required frequency.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.

Observed missing thermometer in walk in cooler. A food thermometer was not readily accessible.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no receptacle in women's room for sanitary napkins. Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
