3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed food being stored on the walk in cooler floor due to the freezer not working at the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Corrected During Inspection Observed the ice shoot in the self service area to have build up. PIC placed an out of order sign on the machine until he could clean the equipment. Equipment food-contact surfaces or utensils are unclean.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Observed restock chicken in the walk in fridge that was dated inproperly. The chicken was dated for being frozen, since their feezer was out of service, I explained that needed to redate for 7 days from the date of cooling. PIC stated he would redate or disgard the chicken. Refrigerated, ready-to-eat, TCS foods not properly date marked.

3717-1-05.1(D) / Backflow prevention - air gap.
Critical Corrected During Inspection Did not observe an air gap on the mop sink hose at the time of inspection. PIC took hose off and stated he would only attach when necessary. Insufficient air gap between the flood rim and the water supply inlet.

3717-1-04.1(Y) / Temperature measuring devices.
Corrected During Inspection Did not observe a thermometer in the mac and cheese warming unit. PIC placed a new thermometer in the unit. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed a torn gasket on the mini fridge by the fries. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed build up on the vents in the walk in cooler. Non-food contact surface(s) not cleaned at the required frequency.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed build up on the microwaves at the time of inspection. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(F) / Drying mops.
Corrected During Inspection Observed mops not hung to dry. PIC hung mops on site during the inspection. Mops dried improperly.

Comments
Freezer was broken at the time of inspection. PIC stated the freezer broke two days ago and they moved all their items into the walk in cooler until the freezer could be fixed. Explained to PIC about refreezing foods that have been thawed out, and the proper date marking for items that have been dated for a frozen state, but now was thawed in their walk in. A re-inspection will take place to ensure the freezer is in working order and the food has been moved off the floor of the walk in cooler.
