Critical Corrected During Inspection Observed buildup of dried mix in cappuccino machine. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed condesnation in bottom of kitchen prep make table. Vent also had some dirt in debris in it. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed missing knob on gas stove. Equipment components are not intact, tight or properly adjusted.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Observed buildup of dirt and debris on shelves in walk in cooler. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Repeat Observed dead flies in light covering. Observed the presence of dead pests or insects.
Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
