Repeat Observed torn gaskets on the walk in freezer and the duke prep table at the time of inspection. Equipment components are not intact, tight or properly adjusted.

Observed holes in the dry wall wall and missing FRP board by the mop sink area at the time of inspection. Walls not smooth and easily cleanable.
