Critical Corrected During Inspection Observed open box of food stored below dripping condensation in the walking freezer. PIC removed it. Talked to PIC about covering all food items to prevent contamination and not storing any food items below dripping condensation until it has been fixed. Food not properly protected from contamination by separation, packaging, and segregation.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed ice build up and condensation in the walk in freezer. Equipment components are not intact, tight or properly adjusted.

3717-1-06.4(A) / Repairing.
Repeat Observed damaged floor tiles by the the floor drain by the dishwasher with statnding water. Talked to PIC about repairing the floor tiles. Physical facilities not maintained in good repair.
