[?] A summary of the violations found during the inspection are listed below.

3717-1-04.4(N)(4) / Manual and mechanical warewashing equipment, chemical sanitization (other) - proper sanitization achieved
Critical Repeat Corrected During Inspection The license holder did not demonstrate achieved sanitization while using a solution other than chlorine, quaternary ammonium or iodine. Observed the mechanical dishwasher that was not dispensing sanitizer at the proper level. PIC stated EcoLab will be contacted to fix the issue and all dishes will be done using the 3 compartment sinks in the meantime.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Corrected During Inspection Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed cole slaw in the walk in cooler that was made on 11/26 and was not date marked. PIC voluntarly date marked it.

3717-1-03.2(D) / Food storage containers - identified with common name of food.
Repeat Working food containers not properly labeled. Observed several food items that were not labeled with the common name.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Equipment and/or components not maintained. Observed excessive ice build up in the walk in freezer due to a leak. PIC stated the damaged part has been ordered to be replaced.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed missing floor tiles by the dishwasher.

Comments
Complaint stated food is served undercooked and not at the proper temperature. That kitchen staffs are not wearing hair restraint, bear restraint or gloves and that hair was found in food several times. That drinking cup are brought back mixed up when taken to serve juice. That cleaning supplies and gloves are not provided for cleaning purposes and that there is an issue of bed bugs. Observed all people working in the kitchen wearing hair restraint, bear restraint and gloves. Observed food hot holding at 135 or above and cold food cold holding at 41 or below. PIC stated cleaning supplies and gloves are available throughout the day for cleaning purposes and that there is no issue of bed bugs at the moment or recently. PIC stated all inmates are assigned they own cup and that they get their juices themselves. No violation regarding the complaint at the time of inspection.
I - P - Employee Health: The operation had an employee health policy on file.
III - P - Preventing Contamination by Hands: Food employees were not contacting exposed ready-to-eat foods with bare hands.
IV - P - Demonstration of Knowledge: The person in charge is Certified in Food Protection.
VI - TCS Food: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked.
VII - Protection from Contamination: Equipment food-contact surfaces and utensils were not being sanitized. The dishwasher is not dispensing the proper level of sanitizer. The 3 bay is being used as alternative way to wash, rinse and sanitize dishes.
