Repeat Facility did not have an ODH certificate in the facility during the time of inspection. Facility does not have an employee with manager certification in food protection.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed wiping cloths to not be stored inside sanitizer water during the time of inspection. Improper use and/or maintenance of wiping cloths.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Observed bathroom sink caulk to be damaged, thus allowing room for debris build-up, during the time of inspection. Fixed equipment not properly sealed or spaced for cleaning.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Observed absorbent cardboard to be sitting beneath the fryers during the time of inspection. Nonfood-contact surfaces constructed of unapproved materials.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Observed equipment used to remove pizza from ovens on pans from the oven to be unclean during the time of inspection. Non-food contact surfaces of equipment are unclean. PIC cleaned equipment during the time of inspection.

Observed several damaged ceiling tiles throughout the facility during the time of inspection. Physical facilities not maintained in good repair.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed coving where front counter meets floor to be damaged during the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.
