Critical Observed PIC not know the proper sanitizing concentration at the time of inspection. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters.

3717-1-02.4(B)(2)(k) / PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
Critical Repeat Observed PIC incorrectly testing sanitizer in the 3 bay sink. PIC unable to demonstrate knowledge of cleaning and sanitizing.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Corrected During Inspection Did not observe an employee health policy on site at the time of inspection. Gave PIC a copy of our employee health policy for them to use. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

3717-1-05.1(O)(1) / Using a handwashing sink - accessible at all times
Critical Repeat Observed handwashing sink next to the donuts to not be accessible at the time of inspection. Handwashing sink not accessible.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed three bay sink stopper in hand sink along with a lemon slice in the drain of the handwashing sink. Handwashing sink is being used for purposes other than handwashing.

3717-1-04.4(N)(3) / Manual and mechanical warewashing equipment, chemical sanitization (quaternary ammonium) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Observed no santizer solution in the 3 bay sink at the time of inspection. PIC corrected this by making new sanitizer water. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness.

3717-1-07.1(A) / Poisonous or toxic materials: Storage: separation.
Critical Corrected During Inspection Observed chemical bottle sitting on top of unopened container of food. Employee corrected this by placing bottle with other chemicals in their designated area. Improper storage of poisonous or toxic materials.

3717-1-02.4(A)(2) / PIC - Manager Certification
Did not observe any employee in the facility with Manager Certification in food protection or the ODH certificate. Facility does not have an employee with manager certification in food protection.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Obsered broken paper towel dispenser at the hand sink towards the back of the kitchen at the time of inspection. Could not get any paper towels out of dispenser at the time of inspection. No towels or drying device at the handwashing sink(s).

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Observed hand sink towards the bank of the kitchen to not have adequate hot water at the time of inspection. Handwashing sink water below 100°F.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed multiple food items around the kitchen and in the walk-in cooler to be stored on the ground. Improper storage of food items.

3717-1-03.2(M) / Wiping cloths - use limitation.
Corrected During Inspection Observed wiping cloths not stored in sanitizer buckets. Improper use and/or maintenance of wiping cloths. PIC corrected this by throwing them away.

3717-1-03.2(K) / In-use utensils - between-use storage.
Corrected During Inspection Observed ice scoop handle to be sitting within the ice during the time of inspection. Employees corrected this by placing it properly.In-use utensils improperly stored.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed coffee filters to be improperly stored at the time of inspection as they were facing up exposing them to debris and bacteria. [51] Improper storage of cleaned equipment, utensils, and laundered linens.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed two torn gaskets in the front two coolers in the front of the kitchen. Equipment components are not intact, tight or properly adjusted.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Repeat Observed litter and debris in the dumpster holding area. Improperly cleaned storage area for refuse, recyclables, or returnables.

3717-1-05.4(O) / Using drain plugs.
Repeat Did not observe a drain plug on the dumpster. No drain plugs in waste receptacle.

3717-1-06.4(F) / Drying mops.
Repeat Observed mop being stored in dirty mop water near the front of the facility at the time of inspection. Mops dried improperly.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Observed entire facility to be un-clean. Every floor needs cleaned in the kitchen, walk-in cooler, and freezer. Facility not maintained clean.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed entire cold holding unit towards the back of kitchen to not be functional and contain several items on top that were unnecessary and cluttering as well. Unnecessary or nonfunctional items and /or litter on premises.

3717-1-06.3 (C)(2) / Employee accommodations - designated areas: lockers
Observed every cooler in the facility to have personal drinks and food. Also observed personal items on shelving above prep area to the left of walk-in cooler.
