| Complainant stated being at the facility at the facility on 6/21 (3 days ago) and observed feces on the wall of the men's rest-room and the floor not being clean. Complainant went back at the facility on 6/23 and observed the men rest-room has not been cleaned. We talked to the PIC about the complaint. Observed the walls in the men's rest-room dirty at the time of inspection. PIC cleaned it during inspection. Reviewed cleaning frequency of restrooms with PIC. |
