Critical Corrected During Inspection Observed food residue on cleaned dishes during the time of inspection. Equipment food-contact surfaces or utensils are unclean. PIC had equipment taken back to dishroom for cleaning.

3717-1-02.4(C)(17) / PIC: duties - ensure the facility has written procedures for responding to vomiting and diarrheal events.
Corrected During Inspection Did not observe a written procedure for responding to vomiting or diarrheal events during the time of inspection. No written procedures for responding to vomiting or diarrheal events. Supplied facility with policy before leaving.

Observed three hand sinks on serving line to not be able to produce hot water during the time of inspection. Handwashing sink water below 100°F.

3717-1-06.1(M) / Outer openings - protected.
Repeat Observed receiving door and kitchen screen door to have damaged door sweeps during the time of inspection. Outer opening not protected.

3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Repeat Observed dead insects and droppings throughout the facility during the time of inspection. Presence of dead pests or insects.

Repeat Observed containers throughout the facility to have dents in them during the time of inspection. Equipment components are not intact, tight or properly adjusted.

Observed clean dishes to be sitting on rags during the time of inspection. Nonfood-contact surfaces not easily cleanable.

Observed ice machine to have a damaged piece on the interior during the time of inspection. Equipment components are not intact, tight or properly adjusted.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Observed there to be no hot water testing device for mechanical dish washing machine during the time of inspection. No sanitizer test kit available.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Observed three bay sink in warewashing area to be leaking and a hand sink at the serving line to be leaking during the time of inspection. Plumbing system not properly maintained or repaired.

Comments
Received complaint requesting that kitchen be inspected for rodents and pests. During the time of inspection I did note a level of rodent droppings throughout storage areas. PIC stated that new door sweeps and gaskets that are better for pest management are to be installed through their new licensed pest control agency. Along with this, I did note a dead insect in a cooler during the time of inspection. These two violations were noted under section 45 3707-1-06.1 (m) and section 45 3703-1-06.4 (L).
