3717-1-04.4(N)(1) / Manual and mechanical warewashing equipment, chemical sanitization (chlorine) - temp., pH, concentration, and hardness
Critical Corrected During Inspection Observed the Chlorine sanitizing solution at incorrect temperature and/or concentration at the 3 compartment sink. PIC added more sanitizer to meet the required concentration.

3717-1-03.4(I)(2) / Time as a public health control - four hour time limit
Critical Corrected During Inspection Improper use of time as a public health control (4 hours). Di not observe an expiration time on the shredded lettuce and the cut onions. PIC stated it has just been brought out and put an expiration time on both containers.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have an employee with level two certification in food protection. PIC has Level 2 ServSafe but did not have the ODH Certificate at the time of inspection.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Employee eating, drinking, or using tobacco in non-designated area. Observed employee drink stored on top of the ice machine.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Food thermometer not readily accessible. Did not observe a thermometer in the walk in cooler and in the coolers in the kitchen.

3717-1-03.2(M) / Wiping cloths - use limitation.
Corrected During Inspection Improper use and/or maintenance of wiping cloths. Observed wiping cloths stored in buckets with sanitizer that do not meet the required level of concentration. PIC had an employee change the sanitizer solution in the buckets.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed build up and debris inside the gaskets of the 2 doors freezer in the kitchen as well as inside the gasket of the cooler next to the freezer. Also observed build up inside the microwave.

Waste receptacles not covered properly. Observed the dumpsters lids open.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed area around the dumpsters needing cleaning.

VI - TCS Food: Observed improper use of time as a public health control for up to four hours. Expiration time was not observed on shredded lettuce and cut onions.
VII - Protection from Contamination: Equipment food-contact surfaces and utensils were not being sanitized. Required concentration of sanitizing was not reached.
