3717-1-06.4(K)/Controlling pests.
Observed many flys throughout the kitchen and three compartment sink area at the time of inspection. Presence of live insects, rodents, and other pests.
3717-1-06.4(F)/Drying mops.
Observed mops stored improperly for drying at the time of inspection. Mops dried improperly.
3717-1-05.1(O)(1)/Using a handwashing sink - accessible at all times
Observed small step stool blocking the already tiny area for the handwashing sink at the time of inspection. Handwashing sink not accessible.
3717-1-03.4(I)(2)/Time as a public health control - four hour time limit
Observed lettuce, tomatoes, cheese, and mayonaise all without proper time stamps at the time of inspection. Improper use of time as a public health control (4 hours).
3717-1-02.3(C)/Hair restraints - effectiveness.
Observed two employees with no hair restraints at the time of inspection. Food employee(s) not wearing a hair restraint.
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Observed microwave with a build up of debris at the time of inspection. Non-food contact surfaces of equipment are unclean.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed vents in walk ins and fan in the kitchen with accumulation of debris at the time of inspection. Observed dirty gaskets on upright Traulsen freezer at the time of inspection. Facility not maintained clean.
Comments
PIC had three new EcoLab lights installed and had EcoLab spray a kill on contact spray safe for food to control the fly population. At the time of follow up there were no flys. PIC installed racks to hang up the mops, implemented a verifiable employee health policy. Still have not heard back from ODH for the half sheet certification, but there are two employees who have ServSafe level two certifications up to date. Delay in follow up inspection was due to training a new food inspector.
