Observed hand sink near the office to not be sealed to the wall properly during the time of inspection. Fixed equipment not properly sealed or spaced for cleaning.

Observed hand sink by the entry door to not be draining properly during the time of inspection. Plumbing system not properly maintained or repaired.

Observed damaged floor tiles throughout the facility during the time of inspection. Physical facilities not maintained in good repair.
