[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Observed milk sitting at the bottom of the mocha machine. To prevent cross contamination this machine should be cleaned after every use. Food equipment surfaces are not cleaned at the required frequency. PIC stated employees rinse the machine with water.
Equipment food-contact surfaces and utensils shall be cleaned at a frequency specified in this rule.

3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
The front cooler has a crack in the glass. This may cause serious injury to a worker. PIC stated co worker called headquarters for a new glass. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed the Delfield cooler with a bad seal as well as the back reach in cooler has a bad seal as well.. The Walkin deep freezer also has ice build up on the roof as well as on the floor. PIC stated will call the company to fix. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed debris on the floor in both dry storage areas. While also the reach in freezer for the hamburger needs to be wiped clean. Non-food contact surfaces of equipment are dirty.
Nonfood-contact surfaces of equipment shall be kept clean.
