Critical Repeat Corrected During Inspection Observed a drink glass to be in the hand sink at the bar during the time of inspection. Handwashing sink is being used for purposes other than handwashing. PIC removed drink glass during the time of inspection.

Critical Corrected During Inspection Observed dented cans in storage during the time of inspection. PIC stated there was no policy for removing, returning, and or disposing of these cans during the time of inspection. Food package(s) received in poor condition. Discussed that items received should be inspected by PIC to ensure they are safe to consume during the time of inspection.

Critical Corrected During Inspection Observed ice scoop handle to be touching ice at the bar during the time of inspection. Food in contact with an unclean surface or linens. Handle was removed from ice during the time of inspection.

Critical Corrected During Inspection Observed food throughout the facility in each cold holding unit and hot holding unit to not have a lid on them during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC had employees place lids on every item during the time of inspection.

Critical Corrected During Inspection Observed raw chicken being stored in walk-in cooler above raw beef during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC moved raw chicken to bottom rack during the time of inspection.

Critical Corrected During Inspection Observed to-go salsa containers to not be held at the proper temperature during the time of inspection. TCS foods not being cold held at the proper temperature. PIC disposed of salsa containers during the time of inspection.

Critical Corrected During Inspection Observed TCS food in walk-in cooler that was prepped on 9/5 still being held in cooler during the time of inspection. Ready-to-eat, TCS food not properly discarded when required. PIC disposed of items during the time of inspection.

Critical Corrected During Inspection Observed bleach being stored near food items during the time of inspection. Improper storage of poisonous or toxic materials. PIC moved bleach to proper area during the time of inspection.

Corrected During Inspection Observed personal employee item being stored in ice for drinks during the time of inspection. Employee eating, drinking, or using tobacco in non-designated area. PIC removed item during the time of inspection.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Repeat Corrected During Inspection Observed hand sink near the warewashing area to not have paper towels during the time of inspection. No towels or drying device at the handwashing sink(s). PIC placed paper towels at sink during the time of inspection.

3717-1-06.1(M) / Outer openings - protected.
Observed back door sweep to be damaged during the time of inspection. Outer opening not protected.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed several wiping cloths throughout the facility to not be stored in sanitizer buckets during the time of inspection. Improper use and/or maintenance of wiping cloths.

Repeat Observed torn gasket on the make table near the grill during the time of inspection. Equipment components are not intact, tight or properly adjusted.

Observed bottom self of table to be entirely covered in rust during the time of inspection. Non-food contact surfaces of equipment are unclean.

Observed hand sink to have ductape keeping parts of it together during the time of inspection. Plumbing system not properly maintained or repaired.

Repeat Observed several damaged ceiling tiles and floor tiles throughout the facility during the time of inspection. Physical facilities not maintained in good repair.

Observed two hand sinks to no longer be caulked to the wall during the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.

Repeat Observed two pieces of food equipment in the back of facility to no longer be in use during the time of inspection. Unnecessary or nonfunctional items and /or litter on premises.Items no longer in use should not be stored in the facility.
