[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(J) / Food contact with equipment and utensils
Critical Corrected During Inspection Observed flour scoop and handle to be sitting inside the flower during the time of inspection. Food in contact with an unclean surface or linens. PIC removed scoop from tub of flower.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed lettuce in cooler to not have a lid during the time of inspection. Food not properly protected from contamination by separation, packaging, and segregation. PIC placed lids on lettuce containers.

3717-1-04.5(B)(1) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Corrected During Inspection PIC stated pizza cutters equipment for removing pizza from oven are not cleaned every 4 hours during the time of inspection. Food equipment surfaces not cleaned at required frequency. Stated these items need to be cleaned every 4 hours and explained why. PIC agreed to begin cleaning them every 4 hours.

3717-1-02.4(A)(2) / PIC - Manager Certification
Did not observe ODH manager certificate in the facility during the time of inspection. Facility does not have an employee with manager certification in food protection. Gave PIC paperwork to receive certificate.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Observed bathroom sink to have damaged coving causing it to be slightly detached from the wall during the time of inspection. Floor and wall junctures not properly coved or closed and/or floor drains not provided.
