3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Corrected During Inspection Observed melted plastic on the fan vent cover in the bottom oven. PIC took the plastic off and had an employee clean it out thoroughly. Non-food contact surfaces of equipment are unclean.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of dust on the vent covers outside of the hood in the kitchen area at the time of inspection. Non-food contact surface(s) not cleaned at the required frequency.

3717-1-06.4(A) / Repairing.
Repeat Observed CresCor Roast-N-Hold equipment not working and not currently in use at the time of inspection. PIC stated they are waiting on a part to repair it. Physical facilities not maintained in good repair.

Repeat Observed damaged/broken floor tiles throughout the kitchen and storage areas at the time of inspection. And water damaged and cracked ceiling tiles in the dry storage area and on the new south line. Physical facilities not maintained in good repair.
