3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Observed build up in the ice shoot of the drink machine. Equipment food-contact surfaces or utensils are unclean.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Corrected During Inspection Observed shredded chicken and pancake butter made on 12/9 without date marking in the 4 door coller. PIC date marked them. Refrigerated, ready-to-eat, TCS foods not properly date marked.

3717-1-07(B) / Poisonous or toxic materials: Working containers - common name.
Critical Corrected During Inspection Observed a spray bottle of sanitizer at the bar without label. PIC labeled it. Working containers of poisonous or toxic materials not properly labeled.

3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of live rodents during the inspection. Observed 2 mice inside the mop closet during inspection. Talked to PIC about getting a pest control company in the facility. PIC is given 24 hours to have a pest control company come and assess the issue and take steps into resolving it. A reinspection will be conducted and proof that a pest control company was at the facility shall be provided. Presence of live insects, rodents, and other pests.

3717-1-04.2(G)(1) / Food temperature measuring devices - availability
Did not observe a thermometer in the 4 door cooler and in the hot box in the kitchen. Food thermometer not readily accessible.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed the ice machine lid missing and ice is not protected from contamination. Improper storage of food items.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed the ice machine without lid. PIC stated the lid has broken and has not been fixed. Observed several damge gaskets on the door of the coolers and freezers. Observed water condensation at the bottom of the 4 door cooler. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(J) / Rinsing procedures.
Corrected During Inspection Observed the 3 compartment sink at the bar set improperly. 3 bay was set with wash and sanitize solutions instead of wash, rinse and sanitize. PIC had an employee reset the 3 bay properly. Utensils and/or equipment not properly rinsed.

3717-1-04.2(I) / Sanitizing solutions - testing devices.
Repeat Observed no sanitizer test kit available. PIC stated test kit was discarded because it got water damaged. Test kit needs to be replaced.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed build up and debris inside the gaskets, on the shelves, on the doors and on the inside of coolers, freezers and hot box throughout the facility. All equipment need cleaned. Non-food contact surfaces of equipment are unclean.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed the floor throughout the kitchen, dry storage and mop closet with build up and debris. Floor needs to be clean and clean underneath equipment. Facility not maintained clean.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Observed the light not working in the dry storage. Light bulb needs replaced. Light intensity less than ten foot candles in required areas.
