3717-1-08.3(H) / Special requirements: bulk water machine criteria - Sampling
Critical Repeat Corrected During Inspection Semi-annually water samples were not able to be provided. When available please fax to Ryan Allarding at Marion Public Health, 740-383-2251. Water tests were faxed to the office. Were complete 4/10/15 and 10/1/15
To prevent illness, the water dispensed from bulk water machines shall be sampled at least semi-annually for coliform bacteria. Sample results shall be retained at the FSO or RFE for review by the licensor.

3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Observed employee drinks stored on top of eggs and in the shrimp case.
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result.
3717-1-03.5(A) / Standards of identity
Observed ham lables in the deli that does not correctly state the product being sold. (brown sugar ham with natural juices, honey ham with water added etc).
Packaged food shall comply with standard of identity requirements in 21 C.F.R. 131-169 and 9 C.F.R. 319, and the general requirements in 21 C.F.R. 130.

Observed the big freezer with excess condension and ice build-up. PIC said he hopes this freezer is re-build in January/ February..
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Observed the cheese self-service case with visible build-up in the dairy department.
Nonfood-contact surfaces of equipment shall be kept clean.

Observed no handwashing sign(s) posted at handwashing sink(s) used by employees in the men's and women's restrooms.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
