3717-1-08.3(H) / Special requirements: bulk water machine criteria - Sampling
Critical Water dispensed from bulk water machine has not been sampled at the required frequency. Water was tested 10/1/15 and 10/12/15. A water test should be sampled at least semi-annually (every six months).
To prevent illness, the water dispensed from bulk water machines shall be sampled at least semi-annually for coliform bacteria. Sample results shall be retained at the FSO or RFE for review by the licensor.

3717-1-03.5(C)(2) / Food labels - packaged food
Observed peanut butter moose, cherry delight, ambrosia, deviled egg potato salad, cut turkey and cut salami without the ingredients statement.
Foods shall be labeled as specified in this rule.

Corrected During Inspection Observed the ingredient book for the donut's in the self-service case on top of the case, inaccessible to the consumer.
Bulk food that is available for consumer self-dispensing shall be prominently labeled in plain view of the consumer.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
There is no test kit available for measuring the concentration of the sanitizer in the produce department.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.4(B) / Cutting surfaces.
The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized in the meat department.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.

To ensure proper cleaning of hands, each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Observed no towels or hand drying device at a handwashing sink in the deil.
To prevent contamination by hands, each handwashing sink or group of adjacent handwashing sinks shall beprovided with: Individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.

A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
