[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed beverage dispensing nozzles in self service area with build up of soil and mold. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

3717-1-03.2(R) / Food storage - prohibited areas.
Observed food that was stored in a feezer that is out of service. PIC disposed of it. Observed food stored in an unapproved location.
To prevent contamination, food may not be stored in areas described in this rule.
3717-1-05.4(H) / Toilet room receptacle - covered.
Observed no receptacle in women's room for sanitary napkins. Observed no covered receptacle in women's restroom.
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed build up in walk in cooler vents. The ventilation system was not being properly cleaned or was creating a public health hazard.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
