Comments
After the initial walk-through, it was determined that the state of the facility had too many critical violations to continue operation and the standard inspection. Therefore, Van Creasap, Director of Environmental Health, arrived to evaluate the facility and came to the agreement with myself and the General Manager to do a voluntary closure of the facility for maintenance repairs. The PIC was instructed to call Marion Public Health for an inspection prior to re-opening the facility. Due to potential fire hazards observed, Van called Captain Makowski at the Marion Fire Department. Capt. Makowski is going to conduct an inspection and report his findings to Marion Public Health.

Observed facility performing Level IV activities with a Level III license. Facility will be required to increase license risk level.
