Critical Did not observe a proper air gap on the prep sink in the produce room.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule.

Critical Corrected During Inspection Observed chemicals stored with clean dishes in the meat room. The person-in-charge moved the chemicals. Observed improper storage of poisonous or toxic materials.
To prevent contamination, poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, or single-service or single-use articles by: 1) Separating the poisonous materials by spacing or partitioning; and 2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles or single-use articles. .
3717-1-08.3(H) / Special requirements: bulk water machine criteria - Sampling
Critical Repeat Water dispensed from bulk water machine sample results were not retained at the facility. Please fax the two most recent water results to Ryan Allarding at 740-383-2251.
To prevent illness, the water dispensed from bulk water machines shall be sampled at least semi-annually for coliform bacteria. Sample results shall be retained at the FSO or RFE for review by the licensor.

Observed cracked meat trays in the meat cutting room. These trays can no loner be washed, rinsed, and sanitized properly.
Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Repeat Observed the use of a non-commercial microwave in the bakery. Equipment is not approved by a recognized testing agency.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.

Did not observe a thermometer inside several of the cold holding cases.
A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.

Observed ice build-up/ condensation inside both of the deli coolers. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

Observed the door jam on the raw chicken cooler needing repaired. Equipment and/or components are not maintained in good working order.

Repeat Observed several air vents in the walk-in coolers with visible build-up.
Nonfood-contact surfaces of equipment shall be kept clean.
