Critical Corrected During Inspection Facility has an employee health policy but could not locate signed copies. PIC stated she will have all food handling employees sign sick form and keep in file for next inspection. Employees are not informed in a verifiable manner of their responsibility to report information about their health.

901:3-4-15(B) / Embargoing of food due to adulteration, misbranding, or expired baby formula or baby foods
Critical Corrected During Inspection Observed multiple dented cans of food during the inspection. PIC pulled off what we saw, and stated she would have an associate go through and pull remaining cans. Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired.

3717-1-04.1(Y) / Temperature measuring devices.
Repeat Observed multiple coolers that did not have an internal thermometer. Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed a torn gasket on the bakery freezer and dairy walk in. Equipment components are not intact, tight or properly adjusted.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Observed multiple walk in and reach in coolers to have dust accumulation on the fan vent covers. Non-food contact surfaces of equipment are unclean.
